Evaluation Process

Completion of an electronic evaluation for each CCE is required for awarding course credit. The CCE evaluation form serves as both an educational tool and documentation of CCE participation.

Students will be submitting evaluations of clinical and community experiences electronically via E*Value. Medical students are required to have the evaluation form completed and turned in no later than the next business day after the experience. Student comments will be sent to the faculty anonymously at the end of each academic year.

Faculty and Community Agencies will be submitting student evaluations by paper. Students are to print the from below and take it to the experience. One form is required for each CCE.

Completion of an evaluation form (signed by the instructor or agency representative ) is required for awarding course credit. Medical students are required to have the evaluation form completed and turned in to the CCE office no later than the next business day after the experience. If the form is faxed to the CCE office rather than given to the student to return, then the student must follow up with the CCE office to view the evaluation. Ultimately it is the student’s responsibility to ensure the form is received by the CCE office.
