Transformative Care Continuum
Physician Education Transformed
A Visionary, Focused and Collaborative Path
To meet the growing need for primary care physicians and to provide the best care for patients, the Heritage College and Cleveland Clinic are transforming medical education. Together, we are responding to rapid and dramatic changes in health care delivery with our own dramatic changes to the way physicians are trained. The Transformative Care Continuum (TCC) is an accelerated pathway based on medical competencies that prepares students for careers in family medicine through a continuous experience from medical school through residency and beyond, with deep experiences with patients and health care delivery teams at its foundation. The program's first cohort of students started in August 2018 and are scheduled to complete residency in 2024.
Learn more about the program and its benefits from TCC Director, Dr. Leanne Chrisman-Khawam.
EIGHT STUDENTS. SIX YEARS. ONE PROGRAM.
Is this the path for you?
- Determined to become a family physician?
- Set on staying in northeast Ohio to practice?
- Driven to go beyond the examination room and into our communities, where responsive care begins?
If you answered “Yes!” the Transformative Care Continuum may be a good medical school fit for you.
Our Partner: Cleveland Clinic
Students will be embedded into their Cleveland Clinic site within their first days of medical school, and they will progress at that same site through to graduation, residency and board certification. Cleveland Clinic anticipates hiring successful graduates to serve as physician leaders within its system.
Work in a new high-tech environment with experienced family physicians who practice full-spectrum, community-based primary care.
Gain the experience of a large, tertiary care academic medical center in a supportive environment.
Dr. Isaac Kirstein, dean at our Cleveland campus, and Dr. Leanne Chrisman-Khawam, director of TCC, describe this innovative program.
Accelerating Change in Medical Education

In 2015, the Heritage College was one of 21 medical schools nationwide chosen to join the 11 founding members of the American Medical Association “Accelerating Change in Medical Education Consortium.” Consortium members are creating the “medical school of the future” by transforming how future physicians are trained.
