How to Create and Submit Teaching Activity Forms in the Salesforce Faculty Portal*

*Teaching activity forms are only necessary for activities not included in eMedley.


Instructions:

  • Go to https://ouhcom.my.site.com/faculty/, it will direct you to sign in using your OHIO credentials
  • Select ‘My Teaching Activity Forms’ in the top banner.
  • To create a new Teaching Activity Form request, select the 'Create New Teaching Activity Record’ button at the bottom of the page. Complete the information and select the green ‘Submit’ button in the bottom left hand corner of the screen.
  • Once you have submitted your request, it will automatically email your Chair for approval. Once approved or denied, you will be notified via email of the approval status. At any time, you can log into this same link and view the status of your requests for this academic year.
