Flu Shot Options
Pharmacies, Physician or Health Clinic
Human Resources has partnered with Anthem and CVS Caremark to provide a convenient and free flu shot program. All employees and family members (7 years of age and older) covered by the University’s Anthem/CVS Caremark health plans can receive flu shots from participating pharmacies during their normal operating hours. Other vaccines may be available for a fee, please check with the pharmacy or provider prior to receiving the vaccination.
You and covered family members (of any age) may also obtain flu shots from your physician or health clinic. Anthem will pay 100% of the allowed amount for a flu shot provided by an in-network physician or health clinic. Flu shots provided by pharmacies will be covered at 100% only if the pharmacy is willing to bill CVS Caremark. Employees must present their CVS Caremark identification card to the pharmacy.
CVS Caremark ID Cards
You will need to provide your CVS Caremark ID card to obtain the vaccination. A digital ID card and information on prescription coverage is available through the CVS Caremark app or the CVS caremark website. If you have questions, contact CVS Caremark at 1-888-964-0089.
Schedule a Flu Shot
Answer the screening questions and select your vaccine(s), location, date, and time.
Select vaccines, find a location, and choose a date.
Complete the online Vaccine consent for. No appointment is necessary and several locations are available.
Visit a Walmart Pharmacy for flu shots and immunizations.
