We would like to accommodate the name you requested to appear on your diploma and in the commencement program when you applied for graduation. However, the name listed in our student information system,  [[PS_Name]], does not match the name you indicated should appear on your diploma, [[AFG_Requested_Name]]. We can permit the name you requested to appear on your diploma and in the commencement program only if you provide supporting documentation for the name change. We must receive your name change documentation (see below for valid documentation) no later than Friday, November 5, 2021. If we have not received the documentation by end of day on November 5, 2021, your name will appear in the commencement program and on your diploma as it appears in our student information system (as listed above).

Please submit a copy of either a valid driver’s license and social security card (must submit both), a federal or state issued identification card and Social Security card (you must submit both), a valid passport, marriage certificate, divorce decree, or other court ordered documents to the Registrar Services division of the University Registrar’s Office.
