Address and Tax Form Updates for F & J Visa Holders

NOTE: The information below is only for students on an F or J Visa. 

All other employees needing to update or correct address information in My Personal Information (MPI), should refer to the Address Update page.

Residents of Ohio, Kentucky, and West Virginia:

1. Please download and complete the appropriate form from the links below, based on the state of your new address.

2. Upload the completed documents to University Human Resources via the secure file upload site. 

Please note: Submitted changes will be reflected in MPI by the pay date following completion and upload of documentation.
