Unpaid Leaves of Absence

Faculty at Ohio University may request unpaid leaves of absence for family or medical reasons, military deployment, and personal or professional development. Policy details are contained in the faculty handbook.

To request an unpaid leave, attach supporting documentation to the form linked below and submit the documents to your department chair or school director for review. If approved, the request will be forwarded to the college dean and then to the associate provost for faculty and academic planning for approval.

For leaves falling under Family and Medical Leave Act (FMLA) guidelines, the completed FMLA form and documentation must be attached to the form.
