Biometric Health Screenings

As part of Ohio University's Personify Health program, Anthem members* can earn a $60 incentive by completing a biometric health screening.

How It Works

To receive the $60 incentive, you must be enrolled in Personify Health and complete your biometric health screening. Choose the screening option that works best for you:

  • Participate in an on-campus biometric health screening event, or 
  • Have your primary care provider complete the biometric health screening form. 

*Anthem members eligible to participate include the benefits-enrolled employee and their enrolled spouse or partner.


What is a Biometric Health Screening?

A biometric health screening is an assessment of your blood pressure, BMI, cholesterol levels, and glucose.  These help you to identify early risk factors for chronic conditions like heart disease and diabetes.  Blood is taken from a quick finger stick.  The appointment takes roughly 10-15 minutes.

  • On-Campus Screening Events

    Before You Schedule

    Enrollment in Personify Health is required before scheduling an on-campus screening. After enrolling, there may be a delay before information appears in the screening eligibility system. To avoid delays in scheduling a screening, we encourage you to enroll in Personify Health as soon as possible.

    Before Your Screening

    Fasting for 8 to 12 hours is recommended but not required. If you are not fasting, inform the clinical team at your appointment. 

    Athens Campus

    The School of Nursing will host on-campus biometric health screening events throughout the fall semester in Grover Center Sim Lab E218 on the Athens campus.

    Chillicothe Campus

    The School of Nursing will host a screening event on October 1, 2026, in Bennett Hall in the commons area between Rooms 142-145.

    Eastern Campus

    The School of Nursing will host a screening event on October 19, 2026, in 317 Shannon Hall.

    Southern Campus

    The School of Nursing will host a screening event on October 30, 2026, in Riffe Rotunda.

  • Physician Screening Form

    Schedule an appointment with your primary care provider and have the biometric health screening form completed.

    Physician Screening Form Submission

    Submit your completed Physician Screening Form by fax at 740-593-0170 or by mail or in person at 1 Ohio University, Grover Center E124, Athens, OH 45701. For your privacy and security, email submission is not recommended. 

    Submission Timeline

    Physician Screening Forms are accepted from July 1 through May 31. You will receive confirmation once your form has been received. If you do not receive confirmation, contact wellworks@ohio.edu.

    Physician Screening Costs

    Preventive care received through Anthem network providers is covered at 100%. Services outside the Anthem network or not classified as preventive care may incur costs. If your visit is not eligible as a preventive exam, consider attending an on-campus screening event.

Screening Results

Screening results are provided directly to the participants and will also be added to their Personify Health profile. Please note that it may take several months for results to appear in Personify Health. WellWorks does not retain copies of screening results. Participants are responsible for sharing their results with their healthcare provider, if desired. 

Biometric Health Screening Incentive

Incentives are paid through the paycheck of the benefits-enrolled employee. If the spouse or partner is an Ohio University employee, all incentives are paid through the paycheck of the employee carrying the benefits coverage. 

All incentive payments will be awarded as taxable income in accordance with IRS guidelines. 

Incentive Payment Schedule
  • Health screenings completed July 1 through November 30 will be processed for payment in December and January.
  • Health screenings completed December 1 through May 31 will be processed for payment in June.
