Recommendation for a Reduced Course Load for Medical Reasons
This form is used as a recommendation for a reduced course load for medical reasons.

Reduced Course Load Recommendation Form
This form must be completed by a representative from the student services office in the student's academic college to help the ISSS team determine if the student may be approved for a reduced course load.

This form is for F-1 students seeking permission to engage in Optional Practical Training (OPT), an employment authorization provided by the United States Citizenship and Immigration Services (USCIS).
