Forms

• Petition for Reinstatement (PDF): This form is for students who have been academically dismissed from Ohio University, College of Health Sciences and Professions
• Re-enrollment Form: This form is for students not currently enrolled who want to return to Ohio University
• Satisfactory Academic Progress (SAP) Appeal Form (PDF): This form is used to appeal SAP status.
• Transfer Credit Form: This form can be filled out at the Student Services office (W370 Grover Center).
• Transcript Request Form: You can order your transcripts online using this form. There are also directions if you would like order your transcripts in person.
• Tuition Appeals and Late Drops: Please contact chspss@ohio.edu to determine is you should complete a tuition appeal or late drop.
• Update of Program Form (PDF): This form is used to change your major, or add a minor or certificate. This form can also filled out at the Student Services office (W370 Grover Center).
