Required Pre-enrollment Discipline and Criminal History Review

Ohio University is committed to providing you with an outstanding student experience within a safe learning environment. As part of that commitment, all admitted students must complete the Required Pre-enrollment Discipline and Criminal History Review form to indicate whether they:

  • Have been subject to formal disciplinary action (including for example, but not limited to, suspension or expulsion for academic or non-academic reasons at any high school, post-secondary institution, college or university);
  • Or have been charged with or convicted of a crime that has not been expunged by a court.

An education at Ohio University is about becoming the very best version of yourself, and we want to create access, not barriers. A previous disciplinary action, charge or conviction, does not automatically disqualify applicants from enrollment at Ohio University, but will be reviewed. This review will be conducted independently of the evaluation of your academic eligibility for admission and will be shared only with Undergraduate Admissions or with other officials at Ohio University who have a need to know as part of the review process.

The Required Pre-enrollment Discipline and Criminal History Review form is available in your MyOHIO Student Center. Log in using your activated OHIO ID and password. If you haven’t yet activated your OHIO ID, visit account.ohio.edu. You will need your PID number and birthdate. Allow up to 24 hours for ID activation.

The information collected in the Pre-enrollment Discipline and Criminal History Review form has always been required for enrollment. This information is now being collected separate from the application for admission.

Submission of the form is required by Ohio University, and you will not be able to sign up for Bobcat Student Orientation, if applicable, or register for classes until the form has been completed, reviewed and cleared. If you answer “yes” to any of the discipline or criminal history questions on the form, we may ask you to provide additional information.

While the information has always been required for enrollment, Ohio University no longer collects this information on the application for admission. OHIO transitioned away from collecting this information in the application for admission to ensure applicants that prior discipline and criminal history will not be visible to reviewers determining academic admissibility.

Where is the form located?

How does the Pre-enrollment Discipline and Criminal History Review work?

Applicants will receive a request via email to complete the Required Pre-enrollment Discipline and Criminal History form, available online after admission in MyOHIO Student Center. The form is simple and quick to complete. Complete the form immediately upon request to allow maximum time for processing and review. The form must be completed, reviewed and cleared for enrollment before you will be eligible to sign up for Bobcat Student Orientation (if applicable) or finalize registration for classes on any Ohio University campus or through College Credit Plus or OHIO Online. Your responses will be reviewed to determine whether further review or additional information is required. If your form is approved, you will receive an email notification that you are eligible to proceed with your next steps in enrollment.

What happens if I answer "yes" to any of the questions on the form?

A previous disciplinary action, charge or conviction does not automatically disqualify applicants from enrollment at Ohio University, but will be reviewed. If additional information is requested from you regarding any affirmative responses, you will receive specific instructions via email. Affirmative responses to discipline and criminal history questions and any additional information you are required to submit will be evaluated by a small group, including staff from Undergraduate Admissions, the Division of Student Affairs, the Office of Legal Affairs and the Ohio University Police Department, to evaluate potential risks posed to the University community. Please note: During peak processing seasons, this review process may take up to a month, so please respond to all requests for information immediately. In the majority of cases, admitted students are permitted to proceed with enrollment processes after additional review. For circumstances in which particular risks are identified by the evaluation group, admitted students’ enrollment may be modified with certain restrictions, terms or conditions. Occasionally, based on the risk assessment, admitted students are not permitted to enroll and must reapply at a later date. Upon conclusion of the review, you will receive a letter in the mail regarding your eligibility to enroll.

How will admitted students who have completed the form be notified if additional information is required?

Admitted students who respond "yes” to a disciplinary question will be contacted by Undergraduate Admissions if additional information is required. Admitted students who respond "yes” to a criminal history question will receive a questionnaire from the Office of Legal Affairs.

What happens if the form is not completed?

Admitted students who do not complete this form will not be able to sign up for Bobcat Student Orientation (if applicable) or finalize registration for classes on any Ohio University campus or through College Credit Plus or OHIO Online.

Who has access to information provided in the form?

Responses are visible only to members of the review committee. Admissions application reviewers, academic advisors and other personnel do not have access to the information.
