This application may be used by U.S. freshman and transfer students applying for admission to Ohio University for spring 2025, summer 2025 and fall 2025. All applicants should read the instructions carefully before completing the application, which includes instructions for applying to Honors Tutorial College and University honors programs. This application may be submitted by international applicants to Ohio University for spring 2025, summer 2025 and fall 2025. International students must submit this form detailing the name and relationship of any financial sponsor and the amount the sponsor is willing to provide toward the student's costs. - Nondegree Student Application and Instructions 2025 (PDF)
This application may be submitted by students who wish to enroll in a limited number of courses at the University and do not intend to earn an undergraduate degree from Ohio University. This application may be submitted for spring 2025, summer 2025 and fall 2025. - College Credit Plus Application 2024-2025 (PDF)
This application may be submitted by students who want to pursue dual enrollment for summer 2024, fall 2024 and spring 2025 on any Ohio University campus. - Required Pre-enrollment Discipline and Criminal History Review form (PDF)
All students admitted to Ohio University on any campus, including OHIO Online, are required to submit this form. The information collected in the Required Pre-enrollment Discipline and Criminal History Review form has always been required for enrollment. This information is now being collected separate from the application for admission. A previous disciplinary action, charge or conviction does not automatically disqualify applicants from enrollment at Ohio University, but will be reviewed. The review will be conducted independently of the evaluation of a student's academic eligibility for admission and will be shared only with Undergraduate Admissions or with other officials at Ohio University who have a need to know as part of the review process. - Regional Campus Application 2024 (PDF)
The 2024 application may be submitted by U.S. freshman and transfer students applying for admission to a regional campus for spring 2024, summer 2024 and fall 2024. (International students should use the international application.) - Certificate Application 2025 (PDF)
This application may be used by students who wish to pursue a standalone certificate program (and not a degree) at the Ohio University Lancaster campus for spring 2025, summer 2025 and fall 2025. - Dean of Students Form (PDF)
Students who have never attended a college or university are exempt from completing this form. This form should only be completed by students who answered 'yes' to one of the academic / disciplinary questions under the Education History section of the application. Additionally, students who are not eligible to return to every previously attended institution must submit the form. The form, when completed, should be sent directly to Ohio University's Dean of Students Office. - Secondary School Disciplinary Report Form (PDF)
This form should only be completed by high school students who answered "yes" to one of the academic/disciplinary questions under the Education History section of the application or who have been contacted by Undergraduate Admissions and requested to complete this form. The form, when completed, should be directly sent to Ohio University Undergraduate Admissions from the secondary school official. - Request for Residency Review

This form is for students who have attended Ohio University as degree-seeking (not dual enrollment or other nondegree) students but who are not currently enrolled and would like to return to OHIO.
