In the event an ID card is lost or stolen, you should deactivate it immediately. You can deactivate the lost ID card using eAccounts. Instructions below:
Failure to report the lost card or deactivate it may result in liability for any fraudulent transactions completed using the lost card.
Cards that need to be replaced due to a defect or extended age will be re-issued at no cost as long as the defective card is turned in upon issuance of the new card. A $12 replacement fee applies for cards that have been lost or stolen, and a $5 fee for cards that are damaged due to neglect or abuse with the return of the damaged card. Replacement cards will be prepared for students only upon verification of current enrollment.
Identification cards that are invalidated by name changes (marriage, etc.), change in identification number, or a change status, will be replaced at no cost with the return of the old card. Students must first provide proof of the changed information through the Registrar's Office before their ID card will be replaced. Employees must first provide proof of the changed information through University Human Resources.
Replacement rates and policies are set by One Card Office in consultation with the One Card Advisory Committee and the Office of Budget Planning & Analysis.
