Alexander School District residents' OHIO tax withholding form due Dec. 18

The Alexander Local School District passed an operational levy that will implement a 1 percent school district tax to all its residents beginning Jan. 1, 2020.  

Faculty, staff, and student employees that reside within the district and have not turned in an updated State of Ohio Employee's Withholding Exemption Certificate need to complete a new form to ensure UHR will begin withholdings for this new tax in 2020.

A new form must be completed in its entirety.  Affected employees must indicate that they reside in the Alexander Local School District and include the district’s number (0501) on the public school district of residence line, the third line on form Ohio IT-4 and on the school district number line, also on the third line, respectively. To ensure appropriate withholding for the entire tax year, a new form must be returned to UHR by Dec. 18, 2019.  If you have made updates to your Ohio withholding form during this academic year you do not need to complete another form.

Completed forms should be returned to the UHR Employee Service center in person, by campus mail, or via fax.
