Course Fee Opt-Out

This form is not for Digital Course Materials Fees.  The opt out for Digital Course Materials Fees is available in Blackboard Learn.  More information can be found at OIT Knowledge Base - Opting out of Inclusive Access.

In an effort to protect students and insure their best interest, the U.S. Department of Education published new rules (34CFR668.161-167) via the Federal Register on October 30, 2015. The Program Integrity and Improvement rules require Ohio University to provide students the ability to opt out of course fees associated with books or supplies.

Ohio University reviews all requests for course fees and approves them only if there is a savings to the student or if it is not reasonable for the student to obtain the book or supply themselves. However, students have a right to opt out of the course fee and the student is responsible for obtaining the book(s) and/or supplies on their own. Please check the Course Offerings for the approved course fees. Ohio University will review all course fee opt-out requests and reserves the right to deny any request that may pose compelling health and safety concerns.

Students wishing to opt out of an eligible course/class fee should download the Course Fee Opt-Out Form[PDF] and submit it to the class professor or academic department no later than the second class meeting of the term.

The Academic Department must submit the completed form to the Office of the Bursar, 010 Chubb Hall, scan and email the form, or fax to (740) 593-0767.
