Ohio University Insurance Guidelines

The insurance guidelines below should apply to all suppliers, contractors, consultants and payees doing business with Ohio University.

Level 1 Risks

  • Suppliers/payees at Risk Level 1 are not required to provide proof of insurance/Certificate of Insurance (COI) or State of Ohio Bureau of Worker’s Compensation (BWC) proof. However, any provider physically providing services in the state of Ohio with one or more employees must have Ohio BWC insurance (https://www.bwc.ohio.gov).

  • Level 2 Risks

  • Suppliers/payees at Risk Level 2 are required to provide proof of insurance in the form of a COI with the below insurance levels indicated. The COI provided must name Ohio University as an “additional insured” and meet the following levels:
    • Commercial General Liability coverage for bodily injury and property damage, in amounts not less than $500,000 per occurrence and $1,000,000 in the aggregate. Professional Liability may be substituted.

    If a supplier/payee cannot or will not provide the COI as outlined above, a Warrant of Insurance must be executed and submitted by the supplier/payee.


    Level 3 Risks

  • Suppliers/payees at Risk Level 3 are required to provide proof of insurance in the form of a COI with the below insurance levels indicated. The COI provided must name Ohio University as an “additional insured” and meet the following levels:
    • Commercial General Liability coverage for bodily injury and property damage, in amounts not less than $1,000,000 per occurrence and $2,000,000 in the aggregate. Professional Liability may be substituted.
      Commercial Automobile Liability coverage in the amount not less than $1,000,000.

    If a supplier/payee cannot or will not provide the COI as outlined above, a Warrant of Insurance must be executed and submitted by the supplier/payee.


    Additional Considerations

  • Construction contracts and other transactions that have state or federal insurance requirements must provide a COI showing the supplier/vendor meets those requirements.
  • A self-insured (SI) supplier/payee who can provide sufficient details concerning the financial stability of their SI program can be exempted from these requirements by the Risk Manager.
  • Exceptions and waivers to these guidelines can be approved by the Risk Manager.

  • Insurance Warrant

    In lieu of a COI, a supplier/payee provider may execute the below Warrant of Insurance Exhibit with their contract as proof of said insurance.

    The Warrant of Insurance Exhibit shall include the language referenced below, which should be modified according to the applicable risk level.

    “Supplier/payee certifies and warrants that at all times during the term of this agreement, it shall carry and maintain, at its expense, all necessary insurance coverage, which shall include the following:

      1. State of Ohio Bureau of Worker’s Compensation Insurance (BWC) to the full extent as required by applicable state law.
      2. General and/or Professional Liability insurance coverage for bodily injury and property damage in amounts not less than:
        a. Level 2 Risks $500,000 per occurrence and $1,000,000 in the aggregate.
        b. Level 3 Risks $1,000,000 per occurrence and $2,000,000.00 in the aggregate.
      3. Commercial Automobile Liability Insurance (Level 3 Risks) in an amount not less than $1,000,000.”
