Updated exam scheduling form from Student Accessibility Services

Student Accessibility Services (SAS) has made some recent updates to the exam scheduling form students use to schedule alternate testing. The current revision includes significant improvements to account for growing variations in how students are assessed and provide more precise information for SAS to administer the exams. The new form will go into effect spring semester. If you have copies of a previous version, please discard them and use the current version moving forward.

The current form can be found on the Student Accessibility Services’ website at https://www.ohio.edu/uc/sas/forms-student-handbook.

Just as a reminder, in order to schedule exams through SAS, students are responsible for turning in a completed form to Baker 348 at least one week in advance of the first exam date listed in order to guarantee testing space in SAS. For exams scheduled through SAS, instructors are asked to submit exams to Student Accessibility Services no later than one business day prior to the scheduled exam.
