2011-2012 Dependent Educational Benefit forms due by May 6

If your spouse, child and /or domestic partner plan to attend Ohio University in fall quarter of 2011, please submit an Educational Benefit Request Form, Qualified Dependents 2011-2012 by Friday May 6.
If you have already submitted this form for your spouse, child and/or domestic partner for the upcoming academic year (fall 2011 – summer 2012), there is no need to complete another one.
Educational Benefit forms are available online from the Human Resource website: www.ohio.edu/hr/forms.cfm#e. They are available in both PDF and Word format.
Forms are due quarterly for employees and annually for dependents.
