Confidential Peer Support

DCP 2017 The Diabetes Community Partners provide confidential peer support.  In other words, people living with diabetes provide support to others living with diabetes.  Peer supporters are trained to assist with goal setting, finding resources and providing support in the day to day challenges of living with diabetes.  Peer supporters do not take the place of health care professionals, but can help encourage consistent diabetes self-management such as making healthy food choices or sticking with physical activity goals.

Need Support?

If you have diabetes and would like to receive support from a trained peer supporter, contact us.  You will be matched with someone who will partner with you one-on-one to work on your diabetes management goals. 

Become a Peer Supporter

If you are confident in your diabetes management and want to help others, contact us to learn more about becoming a peer supporter.  Peer supporters attend free training and receive resources to prepare them to serve in this role.  Continuing support and guidance is provided during monthly Diabetes Community Partners meetings.
