Community Health Programs
Diabetes Care
The Heritage Community Clinic has programs to provide diabetes care to those who lack adequate medical and health care alternatives. The free Diabetes Clinic is offered monthly to qualifying low-income, uninsured and under-insured adults, the Diabetes Navigator program offers assistant to patients to manage barriers to care and the vision care program offers diabetic eye screenings.
Mobile Health Clinic
The Heritage Community Clinic has to mobile units that travel throughout southeast Ohio, providing clinics at churches, community centers and schools. The mobile clinics offer immunizations, check-ups and screenings including screenings for blood pressure, glucose and cholesterol.
Adult Health Screenings
Routine screening can help prevent serious health problems. Our health screenings include checking:
- Blood pressure
- Blood sugar (glucose)
- Blood lipids (cholesterol)
COMCorps
COMCorps is an AmeriCorps program, housed in the Ohio University Heritage College of Osteopathic Medicine Community Health Programs. The Diabetes Institute hires a COMCorps member each year to serve as our Student Diabetes Navigator. The Student Diabetes Navigator is placed with the Athens County school districts to cover 14 local schools. The navigator provides overall direction, resources and education to children and teens with diabetes. The navigator is there to address lifestyle barriers to healthy eating, exercise and will interface between patient’s physician appointments, the school and the family.
