Free Flu Shots/ Other Vaccinations Available During the Fair*

We are pleased to welcome Kroger Pharmacy to this years' event. Kroger will provide vaccine shots to employees covered by Ohio University's Anthem/Express Scripts health plans. Please bring your Express Scripts Prescription ID card to receive your vaccine(s).

NEW! Stop by the Kroger Pharmacy counter the week after the Fair with your Kroger card and let them know you received a vaccine, they will add 50 fuel points for each qualifying vaccine you receive to your card!!!

*Those who are age 50-59 may receive the Shingles vaccine with a $50 copay (exact change or cash only); those 60 years of age or older do not have a copay.

Download and print this form, check the appropriate vaccine(s) you wish to receive. Bring the completed form with you to the Fair, this will help expedite the process.
Don't forget your Express Scripts ID card!

For patient comfort it is recommended that no more than 3 vaccines be administered during one visit.

Door Prize Drawings

Door Prize Cards
Everyone who registers to attend the Fair will receive a Door Prize Card and Name Tag. Visit at least 8 vendor tables and get your card stamped. When you are leaving simply place your name tag on the back of the card and return to the registration table to enter to win a door prize!!!

Some participants will hold door prize drawings at their individual table. Please bring business cards or slips with your name, department and contact information if you wish to enter to win these door prizes.
