Dependent Eligibility Verification Program Deadline Extended to 1/10/18

The following message was emailed to all benefits eligible employees on 12/18/17.

As previously announced, Ohio University has partnered with HMS, an independent verification firm, to conduct a Dependent Eligibility Verification Program for dependents covered by our medical, dental, or vision plans. 

The deadline to submit required documentation has been extended to Wednesday, January 10, 2018

(If you have already completed the requirements of the program, you can disregard this message. If you are interested in confirming you have completed the requirements, please call HMS or visit the HMS web site listed below.)

The purpose of this Program is to ensure all dependents enrolled in our plans meet eligibility guidelines so that only eligible dependents are covered.  All Ohio University employees who have a dependent enrolled in our medical, dental, or vision plans are required to participate.

On November 10, 2017 HMS mailed letters to employee homes with detailed instructions on what they need to do to complete the requirements of the program. This includes submitting documents directly to HMS by the deadline date.   HMS recently released reminder letters to individuals who have not completed the verification process.

Please remember that if you fail to provide information requested by HMS your dependents will be declared ineligible for medical, dental, or vision insurance at the close of the Dependent Eligibility Verification Program.

If you have misplaced your initial letter or reference number, or if you have any questions regarding the program, contact HMS at (877) 473-0175 The HMS customer service line is available between 8:00AM and 8:00PM Monday through Friday. Once you have your reference number, you can log in to the HMS site (, access your personal account, download/print your initial letter, answer questions regarding dependents, check the status of your dependents, and/or upload dependent certification documents.
