Beginning in 2006, the State of Ohio required that all candidates for public employment complete a Declaration of Material Assistance/Nonassistance (DMA) form certifying that they have not provided "material assistance" to a terrorist organization.
This requirement has now been eliminated via passage of H.B. 487 (129th G.A.).
Accordingly, Human Resources is removing the DMA form from its forms web page and from recruiting and new hire materials, and Payroll will no longer receive, keep, or maintain DMA forms.
Please remove the DMA form and any and all references to the DMA form from communications to new and prospective employees.
