Optional Forms for Masters and Doctoral students:

  • Copyright Registration TAD Services does not monitor this process. There is no due date, but registration should be completed within five years of document completion.
  • Publication Delay Request Form Sign and return to TAD Services by final clearance deadline.

All forms may be submitted electronically to tad@ohio.edu, faxed to 740-593-4625 or may be dropped off to the front desk of the Graduate College, Research and Technology Center, Room 220, Athens OH, 45701.

If you are completing a professional project or research paper instead of a thesis or dissertation, you may use a template below to format your document, but changes to the title, approval and abstract pages may be required by your department or college. 

LaTeX Template

Latex Guide (PDF) The guide can be used as a "how-to" for using the "Ohio-etd" class file. We recommend reading this first.

LaTeX Template (.zip) This folder contains the template file.tex and the Class file .cls.

This template was not created by a TAD Services staff member, therefore we cannot provide technical assistance as to its use. Please consult a textbook for help or more information.

MS Word Templates

Instructions on the use of all MS Word templates are written in the sample chapters of each document.

Video training on the use of the templates, including solutions to common problems is available.

Word templates for older versions of word that use the .doc extension (Pre-2007) are available by request. Please email tad@ohio.edu to request an older template for your college.
