Student Health Insurance

Ohio University requires all students enrolled in Athens credit hours to maintain an active health insurance policy. The University offers a policy for students who do not have an active health insurance policy. The Student Health Insurance is a 1-year commercial policy provided by United Healthcare Student Resources, and is re-negotiated each academic year. The policy year is effective August 16th through August 15th of the following year. The policy offers predetermined coverage periods that cannot be changed, and are not pro-rated due to late enrollment or early cancellation.

Students enrolled in the insurance will be charged for 6 months of the annual policy (August 16th-February 14th) during Fall Semester on their student accounts, and the remaining 6 months of coverage (February 15th through August 15th) is charged to their student account during Spring Semester. This provides the student (and enrolled dependents) with year-round coverage without having to take classes over the summer to remain eligible for the insurance. Therefore, the premium rate paid Fall and Spring Semesters is for 6 months of coverage each, and not just the active semester dates.  Students must meet eligibility requirements each semester to be enrolled in the Student Health Insurance policy.

The Student Health Insurance  Policy offered by Ohio University, and provided by United Healthcare Student Resources, offers Domestic, International, and Regional Campus students a comprehensive commercial health insurance policy at an affordable cost. The policy meets and exceeds all Healthcare Reform guidelines and required mandates, and offers coverage specifically tailored to Ohio University student needs.

United Healthcare is a paperless company and does not automatically mail policy brochures and ID cards to members. Insured students are required to create an online account with United Healthcare Student Resources to obtain their ID card and manage their benefits. Notification of ID card availability will be sent to the insured student via email from the insurance company after enrollment has been completed.

There is no option to submit enrollment forms directly to the insurance company online. Voluntary enrollments are completed through the University Student Health Insurance Office only, and are accepted by email (studentinsurance@ohio.edu), Fax (740.593.0699) or mail (2 Health Center Drive #227, Athens, OH, 45701).

 

 

Athens Campus
 

Domestic Students (Athens Campus)
Health insurance coverage is mandatory for all Ohio University Athens students. Domestic students registered (prior to the second week of classes) for five (5) or more Athens campus credit hours (including Athens Campus online classes, Dublin and Cleveland HCOM classes; excluding eCampus classes) will automatically be enrolled and billed for a health insurance policy offered by Ohio University, and provided by United Healthcare Student Resources. Students who have an alternate active health insurance policy may decline the University offered insurance and remove the charge for the premium from their student account by completing an online waiver through their My OHIO Student Center prior to the posted deadline. If a waiver is not completed prior to the posted deadline, the University considers the student to be uninsured, and the charge for the Student Health Insurance will remain on the student’s account. International Students
International students are required to purchase the Student Health Insurance policy offered by the University. International students with Visa status J1 and F1 and are registered for (1) or more credit hours are automatically enrolled in the Student Health Insurance  policy offered by the University each semester upon registration. The premium is charged to the student account Fall Semester and Spring Semester if the student is registered for at least (1) Athens campus credit hour. The online waiver is not available for international students.

Athens Voluntary Enrollment

Enrollment After a Qualifying Life Event
Students who have previously waived the Student Health Insurance are eligible to enroll themselves and their dependents on a voluntary basis if they experience a Qualifying Life Event. A change of life event is a specific circumstance that alters your life or an immediate family member’s life. In some cases, the change is made voluntarily, while other changes are involuntary. Common qualifying events include: losing eligibility for health insurance coverage through an employer, marriage, legal separation, death, aging off of a parent’s health policy at age 26 or becoming eligible for Medicaid. Experiencing a Qualifying Life Event allows a person to enroll in the Student Health Insurance pre-determined coverage period (for which they meet the eligibility requirements) after the open enrollment deadline. The coverage period is not pro-rated.  Refunds and reduced premium amounts will not be given for late enrollment or early termination.

Enrollment After Completion of a Waiver
Students will not be automatically enrolled in the Student Health Insurance Policy after completing a waiver during the same academic year, regardless of re-enrollment in the insurance. Students that have previously waived the Student Health Insurance online are required to enroll in the University Student Health Insurance Policy if they are no longer covered by a major medical health insurance policy.  Enrollment forms must be submitted prior to the open enrollment deadline each semester. Regional Campuses & Centers Enrollment

Failure to purchase the required health insurance for international dependents may result in a Hold being placed on the student account by the International Office of Student and Faculty Services. Premium rates for dependent coverage will be charged to the student's account through the Office of the Bursar.

Waiver Information and Instructions

The Student Health Insurance waiver can only be completed online through the student My OHIO Student Center prior to the posted semester deadline.  The online waiver is the only acceptable waiver format.The waiver cannot be completed until the fee for the Student Health Insurance appears on the student account.  Domestic students are only eligible to waive the Student Health Insurance if they have an active health insurance policy, and can provide the policy information for the online waiver. Students who have alternate insurance “pending” approval cannot waive the Student Health Insurance if they cannot prove active coverage. (Medicaid applications may take 90 days to receive approval, and does not meet the requirements for having an active health insurance policy until coverage is effective.)

The Student Health Insurance waiver information is not shared with University departments. Insurance information will still need to be submitted to the appropriate departments to fulfill various requirements. (IE: Athletics, Campus Care, Housing, etc.)

*Completing an online waiver Fall Semester will waive the Student Health Insurance Policy for Fall, Spring, and Summer Semesters of the same academic year.

*Completing an online waiver Spring Semester will waive the Student Health Insurance policy for Spring and Summer Semesters of the same academic year.

*Completing an online waiver Summer Semester will waive the Student Health Insurance policy only for Summer Semester. A new policy year will begin Fall Semester.

  • Sign into My OHIO.
  • On the Accounts tab, choose My OHIO Student Center.
  • In the Finances section , click 'Waive Stdnt Health Insurance.'
  • Follow the instructions to complete the waiver.
  • Wait for the confirmation page to appear and print for your records.

Insurance ID Cards

United Healthcare is a paperless company and does not automatically mail policy brochures and ID cards to members. Once the insurance company processes the enrollments, they will send an email to the student’s ohio.edu address notifying them that the ID card is available. The email will provide insureds with directions to create an online account with the insurance company. This is very easy to do, and requires insureds to simply choose a username and password. The online account allows students to print an ID card or upload a digital copy to a smartphone or tablet. Online accounts cannot be created until the enrollment has been processed by the insurance company, and the insured has received their email from United Healthcare Student Resources.

Is the Student Health Insurance Policy only for treatment received while on Campus?
No. The Student Health Insurance Policy is a commercial policy provided by United Healthcare Student Resources, and can be used nationwide. The policy offers an extensive network of Preferred Providers through the UHC Choice Plus provider network.

 

Does the Student Health Insurance work only while classes are in session?

Why do dependent enrollment forms need to be submitted each semester?
Eligibility for the Student Health Insurance is determined each semester by the student’s registration. The University system operates on an academic calendar, and by the semester. It is not possible to process enrollment forms for future semesters due to eligibility requirements that must be met.

 

How can I determine what physician’s and facilities are In-Network with the Student Health Insurance Policy?
There is a Provider Locator feature on the insurance website that can be used to locate In-Network providers. The network is United Healthcare Choice Plus, which provides a large selection of providers nationwide. Visit www.uhcsr.com/Ohio and select the provider locator link for either medical or behavioral health services. United Healthcare Student Resources Customer Service can be reached at 1.800.767.0700 to assist you in determining if a provider is In-Network.

Where can I obtain my insurance ID card?
United Healthcare Student Resources is a paperless company does not automatically mail ID cards to students. Students will be prompted to create an account online with United Healthcare at www.uhcsr.com/MyAccount  to manage their benefits and obtain their ID card. The online account allows students to print a copy of their ID card, upload a digital copy to a smartphone or tablet, or request United Healthcare Student Resources to mail them a hard copy of the ID card. ID cards that are mailed from the insurance company will be sent to the address the student has listed as their permanent address on file with Ohio University, or on their existing online account with United Healthcare Student Resources. Students may request a digital copy of their insurance ID card by emailing the Student Health Insurance Administrator at studentinsurance@ohio.edu to have a copy of the document emailed to them. ID cards for newly enrolled insureds will not be available until after the enrollment/waiver deadline has passed for the current semester coverage period.

 

Does the Student Health Insurance Policy include Dental Insurance?
Dental treatment is a policy exclusion and has no payable benefits. However, pediatric dental benefits are included in the Student Health Insurance Policy for insured persons under the age of 19. There are specific providers you must receive treatment from in order for the insurance to be utilized. Please see the Pediatric Dental provisions in the policy brochure.

*Injury to a natural sound tooth by trauma is considered a payable benefit with the Student Health Insurance policy for all insured persons. There is no preventative care included for insureds over the age of 19.

 

Can I Purchase a Dental Policy?
United Healthcare Student Resources offers an optional dental policy for purchase at beginning of Fall and Spring Semesters.  The policy can be purchased through the insured’s online account with United Healthcare Student Resources, or by contacting them directly at 1.800.767.0700.  Students may also purchase the dental policy online at www.uhcsr.com/Ohio.
