Student Health Insurance

Ohio University requires all students enrolled in Athens credit hours to maintain an active health insurance policy. The University offers a policy for students who do not have an active health insurance policy. The student health insurance is a 1-year commercial policy provided by United Healthcare Student Resources, and is re-negotiated each academic year. The policy year runs August 16th through August 15th of the following year. Students purchasing the insurance will be charged for 6 months of the annual policy (August 16th-February 14th) during Fall Semester, and the remaining 6 months of coverage (February 15th through August 15th) is charged during Spring Semester. This provides the student with year-round coverage without having to take classes over the summer. Therefore, the premium rate paid Fall and Spring Semesters is for 6 months of coverage, and not just the active semester dates.

The Student Health Insurance Policy offered by Ohio University, and provided by United Healthcare Student Resources, offers Domestic, International, and Regional Campus students a comprehensive commercial health insurance policy at an affordable cost. The policy meets and exceeds all Healthcare Reform Guidelines and required mandates, and offers coverage specifically tailored to Ohio University student needs.

United Healthcare is a paperless company and does not automatically mail policy brochures and ID cards to members. Insured students are required to create an online account with United Healthcare Student Resources to obtain their ID card and manage their benefits. Notification of ID card availability will be sent to the insured student via email from the insurance company after enrollment has been completed.

Athens Campus
 

Domestic Students
Major medical insurance coverage is mandatory for all Ohio University Athens students. Domestic students registered on time for five (5) or more Athens campus credit hours (including Athens Campus online classes; excluding eCampus classes and Dublin location classes) will automatically be enrolled and billed for a health insurance policy offered by Ohio University, and provided by United Healthcare Student Resources. Students who have an alternate active health insurance policy may decline the University offered insurance by completing an online waiver through their My OHIO Student Center prior to the posted deadline. If a waiver is not completed prior to the posted deadline, the University considers the student to be uninsured, and the charge for the student health insurance will remain on the student’s account. Voluntary Enrollment
Domestic students taking (1) or more credit hours are eligible to enroll in the student health insurance on a voluntary basis prior to the open enrollment deadline each semester.  Enrollment forms can be found on the United Healthcare Student Resources website www.uhcsr.com/Ohio or by emailing studentinsurance@ohio.edu. An enrollment form must be completed each semester and returned to the Student Health Insurance Administrator prior to the enrollment deadline. Enrolment forms may be emailed to studentinsurance@ohio.edu or faxed to 740-593-0699. Enrollment forms submitted past the open enrollment deadline may not be accepted by United Healthcare Student Resources.

* Dublin, OH campus students are considered Athens campus, but are not automatically assessed the insurance charge, and must voluntarily enroll each semester.

 

Enrollment After Completion of a Waiver
Students will not be automatically enrolled in the health insurance policy after completing a waiver for a previous semester during the same academic year. Students that have previously waived out of the Student Health Insurance plan offered by Ohio University are required to enroll in the University insurance plan if they are no longer covered by a major medical health insurance policy.  Enrollment forms must be submitted prior to the open enrollment deadline each semester. International Students
International students are required to purchase the student health insurance policy offered by the University. International students with Visa status J1 and F1 and are registered for (1) or more Athens campus (including Athens Campus online classes; excluding eCampus classes and Dublin location classes) credit hours are automatically enrolled in the student health insurance policy offered by the University each semester upon registration.  The year-round policy is charged to the student account in 6-month increments. The premium is charged to the student account Fall Semester and Spring Semester if the student is registered for at least (1) Athens campus credit hour. This allows students to continue to be insured during summer break without having to take Summer Semester classes. The online waiver is not available for international students.

 

Regional Campus Students


Regional Campus students enrolled for one (1) or more credit hours are eligible to enroll in the Student Health Insurance policy offered by Ohio University, and provided by United Healthcare Student Resources on a voluntary basis. The student health insurance policy is the same for Regional and Athens Campus students. The annual coverage period is from August 16, 2015 to August 15, 2016. The year-round policy is charged to the student account in 6-month increments Fall and Spring Semesters. Enrollment forms must be completed and submitted to the Ohio University Student Health Insurance Administrator at studentinsurance@ohio.edu prior to the posted enrollment deadline each semester to be accepted.  Enrollment forms for Regional Campus students can be found at www.uhcsr.com/Ohio or by emailing studentinsurance@ohio.edu.

 

Dependents (Athens and Regional Campuses)

 

Domestic Dependents
Dependents of domestic students enrolled at the Athens and Regional campuses are eligible to enroll in the Student Health Insurance policy on a voluntary basis. Dependents are eligible for coverage during periods in which the student is also enrolled in the Student Health Insurance. Premium rates for dependent coverage will be charged to the student’s account through the Office of the Bursar.  Enrollment forms can be located at www.uhcsr.com/Ohio. You may also request enrollment forms for dependents by emailing studentinsurance@ohio.edu. Dependent enrollment forms must be completed each semester, and returned to the Student Health Insurance Administrator prior to the enrollment deadline.  Late enrollments may not be accepted by the insurance company.

 

International Dependents
Dependents of International Students with a Visa status of F2 or J2 are required to enroll in the health insurance policy offered by Ohio University. Students must complete enrollment applications for their dependents each semester. Enrollment forms can be found online through the United Healthcare Student Resources website www.uhcsr.com/Ohio or by emailing your request to studentinsurance@ohio.edu. Failure to purchase the required health insurance for International Dependents may result in a Hold being placed on the student’s account by the International Office of Student and Faculty Services. Premium rates for dependent coverage will be charged to the student's account through the Office of the Bursar.

Waiver Information and Instructions
 

The student health insurance waiver can only be completed online through the student’s My OHIO Student Center prior to the posted semester deadline.  There is not an alternate acceptable format, and cannot be completed until the fee for the insurance appears on the student’s account.  Domestic students are only eligible to waive the Student Health Insurance if they have an active health insurance policy, and can provide the policy information on the electronic waiver. Students who have alternate insurance “pending” approval cannot waive the student health insurance if they cannot prove active coverage. (Medicaid applications may take 90 days to receive approval, and does not meet the requirements for having an active health insurance policy until coverage is effective.)

 

*The student health insurance waiver information is not shared with University departments. Insurance information will still need to be submitted to the appropriate departments to fulfill various requirements. (IE: Athletics, Campus Care, Housing, etc.)

 

Completing a waiver Fall Semester will waive the student health insurance policy for Fall Semester 2015, and Spring and Summer Semesters of 2016.

Completing a waiver Spring Semester will waive the student health insurance policy for Spring and Summer Semesters of 2016.

Completing a waiver Summer Semester will waive the student health insurance policy only for Summer Semester 2016. A new policy year will begin Fall Semester of 2016

 

Instructions to complete the online waiver:

  • Sign into My OHIO.
  • Select the Accounts tab.
  • Sign into My OHIO Student Center
  • In the Finances section, select Waive Stdnt Health Insurance.
  • Follow the instructions to complete the waiver.
  • Wait for the confirmation receipt to appear; print it for your records.

 

To waive the Ohio University Health Insurance as an Authorized User:

  • Log into https://webapps.ohio.edu/authorized-access/
  • Select Waive Stdnt Health Insurance.
  • Select the fee you wish to waive.
  • Follow the instructions to complete the waiver.
  • Wait for the confirmation receipt to appear; print for your records.

 

 

Insurance ID Cards

United Healthcare is a paperless company and does not automatically mail policy brochures and ID cards to members. There is an Athens Campus waiver/insurance open enrollment deadline each semester. Students who are enrolling in the insurance for the first time during the academic year will experience a delay receiving their insurance ID card. The University does not supply the insurance company with the roster of insured students until after the posted waiver/open enrollment deadline. However, the insurance effective date begins prior to the open enrollment/waiver deadline. When the insurance company receives the roster from the University, they process the enrollments and backdate the effective coverage date according to our pre-determined coverage periods. Insurance is effective prior to receiving the insurance ID card.  Therefore, if you were to call the insurance company prior to the waiver/enrollment deadline, they will not be aware that you are currently covered. However, your coverage will be processed with the start date of the coverage period purchased. Once the insurance company processes the enrollments, they will send an email to the student’s ohio.edu address notifying them that the ID card is available. The email will provide students with directions to create an online account with the insurance company. This is very easy to do, and requires students to simply choose a username and password. The online account allows students to print an ID card, upload a digital copy to a smartphone or tablet, or email the ID card to a provider. Accounts cannot be created until the enrollment has been processed by the insurance company, and the insured has received their email from United Healthcare Student Resources.

There is a Provider Locator feature on the insurance website that can be used to determine In-Network Physicians. The network is United Healthcare Choice Plus, has a large selection of providers nationwide. Does the Student Health Insurance Policy meet the Health care Reform/Affordable Care Act Requirements?
YES! The policy meets and exceeds all Healthcare Reform Guidelines and required mandates, and offers coverage specifically tailored to Ohio University student needs and requests. The policy offers Domestic, International, and Regional Campus students a comprehensive commercial health insurance policy at an affordable cost. The policy is comparable to that of a Platinum level plan, at the cost of less than that of most Bronze level plans offered through on the open market at healthcare.gov

 

Where can I obtain my insurance ID card?
United Healthcare Student Resources is a paperless company does not automatically mail ID cards to students. Students will be prompted to create an account online with United Healthcare at www.uhcsr.com/MyAccount  to manage their benefits and obtain their ID card. The online account allows students to print a copy of their ID card, upload a digital copy to a smartphone or tablet, or request United Healthcare Student Resources to mail them a hard copy of the ID card. ID cards that are mailed from the insurance company will be sent to the address the student has listed as their permanent address on file with Ohio University, or on their existing online account with United Healthcare Student Resources. Students may request a digital copy of their insurance ID card by emailing the Student Health Insurance Administrator at studentinsurance@ohio.edu to have a copy of the document emailed to them. ID cards for newly enrolled insureds will not be available until after the enrollment/waiver deadline has passed for the current semester.

How do I enroll in the University Health Insurance plan if I have already submitted a waiver?
Students will not be automatically charged for the student health insurance each semester if they have completed the student health insurance waiver during a prior semester in the same academic year.  Students can choose to re-enroll in the University Health Insurance plan during open enrollment periods at the beginning of each semester. Open enrollment will begin (2) weeks prior to the start of each semester, and continue through the posted enrollment deadline for each semester. Students must contact the Student Health Insurance Administrator at studentinsurance@ohio.edu to complete the enrollment process.  

 

Can a student or dependent enroll in the student health insurance policy at any time?
Voluntary enrollment is available during the posted open enrollment periods each semester. Students may submit an application for enrollment after registering for classes for the current coverage period early; however, coverage will not be effective until the posted coverage period begins.

Dependents are eligible to enroll only for coverage periods in which the student is also enrolled

Students who experience a life changing event in which they lose their current coverage are able to enroll in the policy late, but will be charged for the entire coverage period.

 

Can students purchase insurance for only a portion of a coverage period?
No. The student health insurance can only be purchased for the predetermined coverage periods. There is no prorated amount for late enrollment.

 

Why do dependent enrollment forms need to be submitted each semester?
Eligibility for the student health insurance is determined each semester by the student’s registration. The University system operates on an academic calendar by the semester. It is not possible to process enrollment forms and assess charges for future semesters.

Why is Dental Insurance not included in the Student Health Insurance Policy?
Pediatric dental coverage is included in the student health insurance policy for insured persons under the age of 19. There are specific providers you must receive treatment from in order for the insurance to be utilized. Please see the Pediatric Dental provisions in the policy brochure.

Injury to a natural sound tooth by trauma is considered a payable benefit with the student health insurance policy for all insured persons. There is no preventative care included for insureds over the age of 19.

 

Does Ohio University offer a Dental Policy?
No. Ohio University does not have a dental policy available. However, an optional dental policy is available for purchase directly from United Healthcare Student Resources at the beginning of Fall and Spring Semesters.  The policy can be purchased through the insured’s online account with United Healthcare Student Resources, or by contacting them directly at 1.800.767.0700. The University has no affiliation with the optional dental policy offered by United Healthcare Student Resources.
