Process for Seeking Employee Accommodations

As an employee, the request for a reasonable accommodation at OHIO may be initiated at any time.  The request may be directed to a supervisor or to University Equity and Civil Rights Compliance (ECRC). ECRC will facilitate the process.

To register with ECRC, please submit the Faculty & Staff Reasonable Accommodation (ADA) Intake Form either by email, fax or mail.  ECRC will review the disability documentation to determine if the employee meets the definition of an “individual with a disability.”

Once it is determined that an employee is qualified, the process requires coordination with the employee and the supervisor to identify potential accommodations that will allow the employee to perform the essential functions of their job.

When seeking an accommodation, keep in mind that: 

  • Accommodations can only be implemented from the time of the request forward; 
  • The determination of a need for accommodations cannot undo previous consequences of poor work performance;
  • All employees must be able to perform the essential functions of their position;
  • Determination of reasonable accommodations is dependent on the specific limitations of the individual’s disability and the specific functions of the job; and
  • Information regarding an individual’s disability is considered confidential under the ADA.

In order to determine eligibility, most employees will be required to have a qualified healthcare provider complete the Medical Inquiry Form.
