Dependent verification required for benefits enrollment


Starting January 2015, Ohio University Human Resources will require the documentation mentioned below for new hires enrolling dependents in health and welfare benefits. 

Existing employees will need to furnish paperwork when making changes to an enrollment choice, such as adding dependents due to marriage.

Required documentation must be submitted to Human Resources within 31 days of the date of hire or within 31 days of a qualified status change. 

Failure to provide required documentation may result in a denial of coverage for eligible dependents.

Required documentation includes the following:

Married spouse

  • Marriage certificate.
  • Married spouses include same sex partners lawfully married in states that legally recognize their marriages.

Domestic partner (same or opposite gender)

  • Ohio University Domestic Partner Enrollment Form.
  • Ohio University Domestic Partner Affidavit.
  • Supporting documentation as required by Domestic Partner Affidavit.

Children birth to age 25

  • Copy of a birth certificate or hospital record naming you or your spouse/partner as the child’s parent, or
  • Copy of court order naming you or your spouse/partner as child’s legal guardian, or
  • Copy of court order of adoption or adoption certificate naming you or your spouse/partner as the child’s parent.

Children age 26 and 27

  • Copy of a birth certificate or hospital record naming you or your spouse/partner as the child’s parent, or
  • Copy of court order naming you or your spouse/partner as child’s legal guardian, or
  • Copy of court order of adoption or adoption certificate naming you or your spouse/partner as the child’s parent,
  • Attest the child is unmarried and,
    • Is a resident in the state of Ohio, or a full-time student at an accredited public or private institution of higher education, and
    • The child is not employed by an employer that offers any health plan under which the child is eligible for coverage, and
    • The child is not eligible for coverage under Medicaid or Medicare.

What constitutes a family status change?
