Facilities Management Requests

Emergency Work Orders should be called in ASAP to extension 1398. Good judgment normally decides what is urgent; e.g. toilet overflowing, power outage, spills (that are deemed hazardous), HVAC issues (rooms are excessively hot or cold), etc. When reporting a problem, be sure to be specific! Include the exact nature of the problem, the location, (including building name and room number), as well as your name and phone number. 


General Work Orders - In order to ensure our campus is maintained properly, we need to work together as a team. If you see a bulb is out, see leaks in plumbing fixtures, broken furnishings, etc., please complete a work order and submit it to Facilities Management.


Work Order Guidelines - With the beginning of each term, there comes quite a few work order requests. In order to be sure your work order is processed in a timely manner, please use the following guidelines when submitting your request:

  • Submit your work order at least one week prior to the event. You should receive an email verifying that the work order has been submitted.
  • Be specific about set-ups and work requests. For example, the number of tables, chairs, etc. Keep in mind there is a limited number of chairs and tables available.
  • Basic classroom configurations cannot be changed.
  • Work order requests should be submitted online at http://www.zanesville.ohiou.edu/helpdesk. You may also access the system through the Ohio University Zanesville web site Faculty/Staff Portal page. Select Facilities Management Work Order System under Forms & Human Resources.

To Submit a Work Order:

If you have not already done so, you will need to create an account. Click on the link, select new user and complete the form. Use your campus email address.
Login, and select "Submit Work Order".
Select your school from the drop down menu (Ohio University Zanesville or Zane State College).
Fill in the form.
Submit the work order
You will receive an email confirming the work order.
We (Kristin and Joe) will also receive an email acknowledging your work order. We will review the work order, assign it to one of our staff, and add notes if necessary.
You will then get another email with updated information; e.g. that the work has been assigned, etc.
Upon completion of the work, you will receive an email stating that the work order has been closed.

We suggest that you setup your account in our work order system even if you don't need to submit a work order at this time! Please let us know if you have problems setting up your account.



Facilities Usage
Requests - A form must be completed for any event (including workshops, club meetings, luncheons, etc.) held on campus.
You can click here for a form to complete and send to Facilities. For assistance, call Kristin at 740-588-1393.


Parking Tags - A Vehicle Registration card must be completed in order to obtain a parking permit for the faculty and staff parking areas. The Facilities department will issue parking tags to OUZ employees, and Zane State Human Resources will issue tags to Zane State employees. Call Kristin at extension 1393 for OUZ replacement tags or to add new vehicles.  

Supply Requisitions - Requisition forms must be submitted to the Facilities office. Delivery worker will deliver supplies. Click Here for form
