Photo Request

Please complete the form below and click "submit" to request photography services.

Professional studio headshots are taken on the first Friday of every month. Alternative days and times are scheduled as necessary. The cost is $40 per headshot. All headshots are taken in the photography studio located in Scott Quad Room 155.

Note: Required fields are in green type.


i.e. Portrait in Studio, Existing head and shoulder photo of President McDavis, Event Coverage, Portrait on Location, Group Portrait in Studio, Group Portrait on Location
Please list the names, e-mail addresses, and phone numbers for all subjects to be photographed. For large group portraits and event coverage, please provide contact information for the individual/s responsible for coordinating the group or event.

Example: If you are requesting a photo in Baker Center, please specify a room number or a landmark, i.e. the windows in front of the Baker University Center Ballroom.

All photos will be provided in JPG format unless otherwise requested. Check this box to request 16 bit color TIFF formatted images.
