Conflict of Interest Form A - You have no relationships or financial interests that are, or could be perceived to be, a potential conflict with your duties and responsibilities at Ohio University AND have no other potential conflict disclosures on file with the office of research compliance. 
 
Conflict of Interest Form B
 - You have a relationship or financial interest that has the potential for, or could be perceived to be, in conflict with you duties and responsibilities at Ohio University.
