Dear Ohio University Faculty and Staff,

After consideration of the recommendations I've received from a number of University constituencies, I have made the following decisions related to health benefits for Ohio University employees beginning July 1, 2009.

Premiums:
- There will be no premium increases.

Deductibles and Requirements:
- $200 Single/$400 Family deductible
- $20 office visit co-pay (previously $15)
- $50 emergency room co-pay
- $1,000 Single/$2,000 Family co-insurance limit (previously $750 Single/$1,500  Family)
- Mandatory generic pharmaceuticals
- Mandatory mail order pharmaceuticals for maintenance prescriptions
- $50 monthly surcharge for Ohio University-insured spouse opting out of other  employer-provided insurance
- When both spouses work at Ohio University, premium rate calculated on highest paid spouse (This requirement eliminates inconsistencies in current practice)

New and Expanded Benefits:
- Increased child eligibility to age 25 if full-time student with $50 monthly surcharge  (previous eligibility ceased at age 23 if full-time student)
- Extension of coverage to legal dependents
- $5,000 adoption benefit for associated fees
- Expansion of domestic partner eligibility to include opposite gender partner  (previously offered only to same gender partner)
- Increased medical lifetime maximum to $3 million (previously $2 million)
- Increased mental health benefits: first five visits paid at 100 percent with $15 co- pay thereafter (previously, visits beyond the first five were covered at 70 percent)
- Expansion of coverage for survivor health care to five years (previously six months)
- Addition of Healthy Ohio benefit that includes health risk appraisal, WellWorks  membership incentive, disease management program and preventive care updates
- Increased flexible spending account annual limit to $5,000 (previously $3,000)

The University expects to complete negotiations with Holzer Clinic on Monday, March 16, 2009. We will notify you, along with Anthem, the University's plan administrator, of the results of those negotiations at that time.
