PURPOSE:

To encourage good health, promote the public health of the academic community, and provide the basis for medical care.

POLICY:

A medical history form will be required of each student requesting services at the time of the first visit to the Hudson Health Center. Students are encouraged to complete the form prior to enrollment at Ohio University. The medical history form is included in the matriculation materials. The form includes a comprehensive medical history and indication of proper immunization against tetanus within the past ten years and history of proper measles, mumps and rubella immunization or illness. Lack of proof of proper immunization status may prevent enrollment in the University or exclusion from the campus should an epidemic arise.

Any student who has not received treatment by Medical Services for four or more consecutive years will be required to complete a new medical history form.

Also required for each international student new on the Athens Campus (or one re-enrolling after an absence of two or more years) is a tuberculosis skin test administered by University Medical Services at the time of entrance to the University. All positive skin test reactors must receive a chest x-ray for evaluation of active tuberculosis. Referral to a prophylactic treatment program will also be recommended for positive reactors.

The above medical requirements will apply to workshops and special program participants and will be implemented in the same manner as for regular students.

Waiver of medical requirements may be granted by the Director of University Medical Services in appropriate cases.
