University Human Resources noted that the PPO coverage chart included in the enrollment packets contained an error. The chart should indicate that inpatient and outpatient services and surgery, and inpatient mental health are subject to the deductible. There are several changes to the benefits plan effective July 1, 2009. To find out more information about the changes, click here.

Such changes include changes to employee premiums, which are based on salary and family size. Click here for a premium rate sheet in PDF format.

Employees covering a spouse or domestic partner may face an additional $50 monthly premium if the spouse or partner is employed and not enrolled in his or her employer's health plan. Click here for more details regarding this $50 premium.

For employees who are married to or a domestic partner of another university employee, premiums will be based on the highest-paid spouse or partner. Click here for more information.

• 1 to 2 p.m. Thursday, May 7, in Baker University Center 231
