Project Requests

Customers Requesting a Project

 

HOW DO I REQUEST A PROJECT?  Anyone wishing to request a project with the Office of Information Technology must fill out one of the following forms:

Project Request Form (PRF) - Use this Excel workbook to request all standard projects including the development of online applications, forms, or interfaces.

Web Request Form (WRF) - Use this Excel workbook to request the development of a new web page or changes to existing web page(s). Note: This does not include the development of online applications, forms, or interfaces. 

WHEN DO I NEED TO FILL OUT A REQUEST FORM?  You need to fill out a request form if either of the following conditions apply:

-- You have been specifically directed to fill out a request form - OR -
-- You believe your request will require 100 hours of effort or more.

When in doubt, fill out a request form.

For small requests, please contact the OIT Service Desk at 593-1222 or submit a request online.

WHAT HAPPENS TO MY PRF?  The PMO will shepherd PRFs through the review and approval processes. (See Project Flow Chart)

HOW DO I  SUBMIT THE REQUEST FORM?  PRFs and WRFs must be fully completed before submission. Attach your Excel document (PRF/WRF) to the online submission form - see submission instructions at the bottom of the PRF/WRF. For further information on completing a PRF, see the Checklist for Customers.
