ATHENS, Ohio -- Ohio University College of Osteopathic Medicine's Community Service Programs continues its summer sports physical clinics for Southeastern Ohio youth with a clinic at the Coolville Healthcare Center in Coolville Friday, July 19, from 9 a.m. to noon. CSP's sport physicals began July 10.
The Coolville sports physical clinic is held in collaboration with the Coolville Healthcare Center. Physicals will be offered for youth, ages 10 to 19.
The cost of each physical is $15, payable at the time of the examination. Immunizations also are available at no charge to youth through age 18 whether receiving physical examinations or not. Ohio High School Athletic Association physical forms will be provided for sports physicals. Camp physical forms need to be provided for children requesting camp physicals. Appointments need not be scheduled prior to the clinic.
Parents must bring immunization records for children to receive immunizations. Children under 18 years old not accompanied by parents must bring immunization records and permission slips signed by their parents or guardians in order to receive physicals and/or immunizations. Permission slips must specify what services children are allowed to receive.
Additional CSP sports physical clinics will be held Wednesday, July 31, and Wednesday, Aug. 7, at the family medicine wing on the second floor of University Osteopathic Medical Center, Parks Hall, in Athens. Each clinic will be from 6 to 8 p.m. CSP also sponsors free immunization clinics throughout Southeastern Ohio in the month of July.
