Use this version if you want to print the application, fill it out, then either fax it (with your credit card information included) or send it by postal mail (with your credit card information, check, or bank draft on a U.S. bank).

Use this form to request an extension of your enrollment. This request must be made before your enrollment period expires. Send it by postal mail (with your credit card information, check, or bank draft on a U.S. bank).
