A diaphragm is a flexible ring covered with thin soft rubber to make a shallow cup.  It must always be used with a spermicidal cream or jelly.  As a barrier method, the diaphragm fits securely in the vagina and covers the cervix.  The cup portion holds the spermicide against the cervix.

There are two types:  arch type or the all flex. The diaphragm comes in different sizes from about 50 mm to 85 mm. The measurement indicated distance from the pubic bone to the cervix.

The diaphragm acts as a container to hold spermicidal cream or jelly against the cervix, and as a physical barrier blocking the passage of sperm from the vagina to the uterus. A good fit is essential. The cream or jelly kills the sperm in the vagina.

The actual use effectiveness rate range is 80-95%.  The effectiveness of the diaphragm is directly related to the woman's motivation not to become pregnant.  Other factors that increase effectiveness of the diaphragm are through education programs, a careful fitting and an insertion practice session with a follow-up visit to check the fit and client insertion ability.

To effectively use a diaphragm, begin by putting about two tablespoons full of jelly or cream into the cup of the diaphragm and spread some around the internal rim.  (Read the directions on the spermicide.)  Fold the diaphragm and slide it in as far back as it will go into the vagina.  Insert your finger into your vagina to check the position of the diaphragm; it would be directly over the cervix.  The cervix feels like a small "o" about the same texture as the tip of your nose.  Most of the time the diaphragm goes into the correct position.  The diaphragm can be inserted up to six hours ahead of time.  It can be inserted at the time of intercourse.  You can insert the diaphragm while standing with one leg up, or in a lying down position.

You must leave the diaphragm in for at least 6-8 hours after the last act of intercourse.  Do not douche while it is in place.  Extra spermicide should be used for each additional act of intercourse.  This can be added with an applicator purchased at the same time as your spermicide.  Due to the risk of Toxic Shock syndrome, the diaphragm should not be left in the vagina for more than twelve hours at a time.

Remember the twelve-hour guideline if you use the diaphragm during menstruation.  Wash it with soap and water; rinse and pat dry after each use.  You can dust it with cornstarch, but do not use powder.  Check it for holes.  Return it to its container and store away from heat.  If the woman gains or loses over ten pounds, she should get the size rechecked.  The diaphragm should be replaced every two to three years; however, the fit should be checked at the yearly gynecological visit.  

Advantages:
  • It is highly effective and has no medical side effects
  • It can be inserted up to six hours in advance or just prior to sexual intercourse
  • * Your partner can participate in inserting the diaphragm and therefore it will not interrupt love making
  • Low cost
  • Is used only when you need it
Disadvantages:
  • Allergic reaction to the rubber or spermicidal cream or jelly.  If this happens, try another brand.
  • Recurrent cystic (bladder) infection due to pressure against the urethra can occur
  • There can be some discomfort if the diaphragm is not the right size
  • It can me messy
  • A slight increase risk for Toxic Shock Syndrome (TSS)
This method requires a prescription.  A pelvic exam to fit the diaphragm properly and education session on insertion, use, care and removal is necessary.  A trained physician should determine the right size for you. Diaphragms are available through the pharmacy in Hudson Health Center.
