About Us
The Student Health Service Laboratory holds a Certificate of Accreditation (COA) issued by the accreditation organization COLA. This accreditation ensures that the laboratory is in compliance with the Clinical Laboratory Improvement Amendments (CLIA) which is published and enforced by the Centers for Disease Control and Prevention (CDC) and the Centers for Medicare and Medicaid Services (CMS).

Location
The Laboratory is located on the ground floor (basement) in Hudson Health Center 011. Call for test results at 740-593-4730 *.  The laboratory fax is 740-593-0179.

* The phone number provided is the Medical Information Desk for SHS.  Laboratories certified under the Clinical Laboratory Improvements Amendments (CLIA) may disclose test results or reports only to authorized health care providers. The laboratory staff at the Student Health Center cannot provide test results directly to patients over the phone or in person. Patients can access their test results by calling the front desk at 740-593-4730 or by going to the Medical Records department on the first floor of Hudson Health Center.

Am I eligible to use the laboratory?
If you are enrolled for one (1) or more credit hours, you can obtain services at the Student Health Service laboratory. You must be enrolled at the time you use the laboratory.

Does the laboratory accept orders from outside physicians?
We accept orders from any physician in the United States. Your physician may write you an order or fax it to 740-593-0179. If the test is to be done on a routine basis, the physician needs to indicate that this is a "standing order" and they should also specify the frequency, i.e. every month, times 3, once a week, etc. Orders from outside physicians are valid for one year after they are written, even if they are "standing orders."
  • Procedure for a student wishing to use an order from an outside physician:
  • Bring the order to the Student Health Service or have the order faxed to 740-593-0179 (Attention: Laboratory)
  • Check in at the front desk on the first floor
  • The first floor staff will collect all pertinent information and make a copy of the order for your chart.
  • The first floor staff will send you to the laboratory with a copy of your order.
  • Present this order to the laboratory staff and sign in on the patient log.
  • The laboratory staff will keep the order for our records
  • If the order is a "standing order" the student only has to follow steps 1-3 their first visit to the Student Health Service laboratory. All succeeding times they may come directly to the laboratory.
  • The laboratory will fax the results to the requesting physician. If the contact information for the requesting physician is not on the order the student should obtain it before coming to the laboratory.
How do I pay for my laboratory procedures?
The charge is placed on your student account and payment can be made in person at the Bursar's office in Chubb Hall or by accessing your online account.

Can I use an insurance card at the Student Health Service laboratory?
We are not able to bill your private insurance. Instead, we will give you an itemized bill to send to your insurance company for repayment according to their schedule. The charge is placed on your student account and payment can be made in person at the Bursar's office in Chubb Hall or by accessing your online account.

How do I get a receipt for my laboratory procedures?
The student can get an itemized statement of charges from the Business Office located on the first floor of Hudson Health Center, Room 111. This statement of charges will include the information necessary to bill your insurance company. If you do not take care of your own insurance, please send this form to your parent or guardian.

Does the laboratory perform drug screens?
The laboratory at the Student Health Service can collect drug screens for pre-employment, court ordered, or any type of physical. We a capable of collecting chain of custody drug screens as well as non-chain of custody drug screens. We do not perform the drug screens in our laboratory. We send them to LabCorp, Quest Diagnostics, or Medtox Laboratories depending on the circumstance. If the drug screen requestor requires that the specimen be sent to a laboratory other than the ones listed, they should provide the student with the appropriate collection kit and shipment information. In this situation the laboratory at the Student Health Service can still perform the collection and shipment of the specimen and a fee is charged to the student for this procedure.

Procedure for having a drug screen at SHS:
  • Bring the order to the Student Health Service or have the order faxed to 740-593-0179 (Attention: Laboratory)
  • Check in at the front desk on the first floor
  • The first floor staff will collect all pertinent information and make a copy of the order for your chart.
  • The first floor staff will send you to the laboratory with a copy of your order.
  • Present this order to the laboratory staff and sign in on the patient log.
  • The laboratory staff will keep the order for our records
  • The student will be asked to show a picture ID so be sure to bring one
  • If we are doing a chain of custody drug collection a laboratory staff member will be present in the lavatory when the specimen is collected
  • The student and the laboratory tech will then fill out the drug collection order form together and the student will be given a copy
  • The laboratory will fax the results to the requesting physician or employer. If the contact information for the requesting physician or company is not on the order the student should obtain it before coming to the laboratory.
