Medical Records/Confidentiality
The Student Health Service is a covered entity based on the Health Insurance Portability and Accountability Act (HIPAA) Privacy Rule. It is our intent to comply with all provisions of the Privacy Rule as outlined in the final draft of this act of Congress.
Student Health Service (SHS) medical records are confidential. While there are exceptions related to legal issues and medical emergencies (see SHS Privacy Notice), information is not released from your medical records to anyone, including parents, for any purpose without your written consent. It is not our policy to release medical information without your authorization, or guidance from the privacy elements of HIPAA. This information is outlined in the SHS Privacy Notice.
If you know a parent or guardian will want to talk to the provider related to your medical care in SHS, you can give permission at the time you are seen. If written chart documentation is to be released, you must complete a "Authorization to Release of Medical Information" form. This can be done in Medical Records, Room 117, Hudson Health Center, or you can print the authorization form from this site [link "Authorization to Release Medical Information"] and bring it to Medical Records, or fax the form to 740-593-0179 (Attention: Medical Records).
