The Ohio University Student Injury and Sickness Insurance Plan (hereafter "student insurance plan") is designed to supplement care provided by the Student Health Services and is required for Ohio University students attending the Athens campus who do not document other insurance coverage. It is provided by Student Resources, a private company under contract with Ohio University. The student insurance plan covers illnesses and injuries that occur either on or off campus, 24 hours a day anywhere in the world. This plan is also available to spouses and children of university students.

It is very important that you read the brochure that explains the benefits, limitations, and exclusions associated with the student insurance plan. Nothing printed in this web site should be understood to be a promise of payment. Students must meet the guidelines as presented in the brochure. The 2008-2009 brochure is available here.
What does the Student Insurance Plan offer? Ohio University, through Student Resources, offers one medical insurance plan for all students. Plan coverage generally begins on September 1st of an academic year and ends at 12:01 AM of the following year (For example, 9/1/08 - 9/1/09). Students will be billed for the insurance on their Fall, Winter and Spring quarter fee bills. These three equal payments will cover the student from 9/1/2008 - 9/1/2009.
Student Insurance Plan
When as the result of an injury or sickness eligible medical expenses are incurred while covered under this plan, the insurance company will pay in excess of the $50 deductible, or $15 co-pay, 100% of eligible expenses up to $5,000 and then 80% of eligible expenses until the member has reached an out-of-pocket maximum of $5,000. The Plan will then pay 100% up to the $50,000 aggregate maximum per injury or illness. The deductible and co-pay are waived for services at the Student Health Service.
Prescription coverage:
- Plan maximum per year is $1000 (Accident / Injury)
- Plan maximum per year is $5,000 (Psychotropic Drugs)
- The student must use SHS pharmacy unless Hudson is closed, or the student is home on break. When Hudson is closed use your insurance identification card at participating pharmacies and pay $10 or $25 copay.
- For psychotropic drugs you will pay for the prescription and send in receipts to be reimbursed. ($15 deductible per prescription and 80% reimbursement)
Students should always use participating providers.
If a student chooses to use non-participating providers, the insurance reimbursement is reduced from 100% to 50%. In Athens, Hudson Health Center is the provider. Elsewhere, participating providers are those listed with United Healthcare(www.uhcsr.com/college/ViewBrochures.aspx, select Ohio University from the drop down list, click the provider search link).
Students will be billed as follows: - Fall Qtr. - $338; Coverage Period 9/1/2008 - 12/31/08
- Winter Qtr. - $338; Coverage Period 1/1/2009 - 4/30/2009
- Spring Qtr. (1) - $338; Coverage Period 5/1/2009 - 8/31/2009
- Spring Qtr. New (2) - $434; Coverage Period 3/30/2009 - 8/31/2009
- Summer Qtr. - $195; Coverage Period 6/22/2009 - 8/31/2009
What services are covered by the plan? Services covered by the plan include (See brochure for details of coverage): - Treatment by legally qualified medical practitioner
- Hospital confinement
- Services of a licensed nurse
- Ambulance service
- Prescription medications for sickness or injury
- Psychotropic prescriptions
- All other medical services and supplies which are ordered or prescribed by the attending physician - unless listed under "Exclusions"
- Alcoholism treatment
- Mental and nervous disorders
- Vision exam
- Repatriation
- Medical evacuation
No payment will be made for expenses incurred by any insured person while enrolled as a student and in attendance on the Athens campus for services which could have been provided by the Ohio University Student Health Service.
How do I get an insurance card and brochure? You can print a copy of the Ohio University Injury and Sickness Insurance Plan Brochure from the Student Resources website or you can pick one up from the student insurance office located in Hudson Health Center 233. Permanent insurance cards are mailed to the student at his or her permanent address approximately two weeks after the last day to waive the insurance plan. The student can receive a temporary card at the student insurance office, Hudson Health Center 233.
If I'm not enrolled in classes, can I still get the Student Insurance Plan? Those eligible include students participating in Ohio University-sponsored internship programs; study abroad programs; co-op programs; masters and doctoral thesis students taking at least 1 thesis hour; graduate students taking at least 3 credit hours; and J1 VISA visitors/researcher scholars. If you fall into one of these categories and you want to buy the student insurance plan, you must complete a Special Category form. This form is only available in the student insurance office at Hudson Health Center 233. For those students being dropped from a parent's insurance coverage, you will be able to enroll in the student insurance plan. You will be required to have a letter from the insurance company indicating that you have been dropped from the plan. You will be charged the premium for the entire coverage period for a particular quarter. You will also be asked to complete a form that reverses the waiver card you had completed either on line or hand written. The student will then be billed automatically by the University for future premium amounts.
How do I file a claim under the student insurance plan? In order to be reimbursed for medical services you have received, the student or the student's dependent must complete either a Student Medical Insurance Referral Form or a Student Medical Insurance Information Form. - A student will use the Student Medical Insurance Information Form for services received in Hudson (including prescription medications).
- The Student Medical Insurance Referral Form that must be completed for all services received outside of Hudson, including prescription medications.
- For each different sickness or accident the insured student will be required to send in either a Student Insurance Medical Referral Form or a Student Insurance Medical Information form. These forms will be mailed directly to United Healthcare Student Resources PO Box 809025 Dallas, TX 75380-9025. The forms and envelopes are available in the insurance office at Hudson Health Center 233 or you can print the forms directly from the United Healthcare website, or the Student Resources website, select Ohio University from the drop down box, and click on the link the reads "2008 Referral Form".

Checklist for filing a claim under student insurance plan: - Gather all your SHS Pharmacy bills, other physician bills, and/or other pharmacy bills. (Insurance companies need the actual listing of the services provided. They will not accept your charge receipt, or your OU account statement.)
- You can obtain a copy of your bill for services at the Student Health Service in the business office at Hudson Health Center 237, or call 740-597-1816. Pharmacy charges can be obtained from the pharmacy at Hudson Health Center 109, or call 740-593-4738.
- For services in Hudson, you will send the pharmacy invoice along with the Student Medical Insurance Information Form directly to UHCSR for sickness and injury prescription's reimbursement. Outside pharmacy receipts should be mailed to Medco except for mental health drugs which will also go to UHCSR.
- For services you receive outside of Hudson (e.g. hospital, physician office, out-patient surgery center, pharmacy), ask if they will bill the insurance company for you. If they bill insurance companies, they will ask you for your insurance card, and they will then send the bill for charges to the insurance company. However, you must still complete the Student Insurance Medical Referral Form. If the provider or the hospital will bill the insurance company for you, you will send the Referral Form separately. UHCSR will match these two documents. If the provider is not sending a claim to the insurance company, then you will attach the bills to the Referral Form and mail them to UHCSR.
- Obtain a Referral Form from the UHCSR web site, or the student insurance office.
- Complete the Referral Form, making sure that you have included all requested information.
- a. If you have been referred for medical care to a medical service provider outside of Hudson Health Center by a Student Health Service provider, a form will be completed by a staff member and electronically filed to UHCSR.
- b. If you had to seek medical care when Hudson was closed or in an emergency or when you were not in the Athens area, you will complete that form yourself by placing a check mark next to the appropriate item.
- For all pharmacy services received in the Student Health Service you will be charged through your university account. You are responsible to pay your bill at Chubb Hall. Students will receive reimbursement checks for prescriptions from the student insurance company. The check will be sent to the address provided on the Information Form, or Referral Form. Please make sure the address is correct.
- Sign and Date the form.
- Use a paperclip or staple to attach all bills and supplemental information to the back of the Student Insurance Referral Form. (If you received medical services outside of Hudson and that provider billed the insurance company for you, you will only send the Student Insurance Referral Form.)
- Mail all documents to the address provided by the insurance company specifically for claim information.

Instructions for the Student Resources Web Site at www.uhcsr.com/college/ViewBrochures.aspx: - Choose "Ohio University" from the drop down menu, and click on "View your plan brochure."
- You can print out Student Medical Insurance Referral forms, the brochure that details your plan, and enrollment forms for dependents.
- If you are away from the Athens area and would like to find a doctor, click on the "Provider Search" link. Your insurance plan does participate with the United Healthcare network. Your coverage is based on your using preferred providers. If you go to an out-of-network provider, your reimbursement will change from 100% to 50%.
- If you are a domestic graduate student or undergraduate student and do not meet the 7 credit hour requirement for automatic billing, consider the Special Category classifications mentioned in the brochure. If you are participating in an internship, study abroad program, co-op program, masters or doctoral thesis program, 3 hours of graduate study, or are a J1 VISA visiting scholar, you may be eligible to purchase the plan. Special Category forms are only available through the student insurance office at Hudson Health Center 233. Call 740-597-1816.
- Students meeting the University eligibility requirements of 7 credit hours for domestic student and 1 credit hour for international student will be billed automatically on your student account. Payment must be made with other University fees.
Vision plan - There is a Vision Plan included in the cost of the student insurance plan. Benefits include:
- One comprehensive eye exam each 12 months at no additional cost.
- 20 % discount off the VSP doctor's fee for prescription lenses when a complete pair of glasses are purchased
- 20% discount off the VSP doctor's fee for frames when a complete pair of glasses is purchased
- Contact Lens Evaluation and Fitting. 15% discount off the cost of your contact lens exam. This exam is in addition to your routine exe exam.
- Contact Lenses - VSP offers valuable savings on annual supplies of certain brands of contacts. Visit www.vsp.com for details.

Instructions for the VSP web site: - Click on the "Members and Consumers" box.
- Follow the sign on instructions. These instructions ask for the last 4 digits of your social security number and your first and last name.
- Once you have signed on, a page opens with information about the benefits associated with the OU student plan. If the VSP system does not recognize you as a subscriber call the student insurance office at 740-597-1816.
- Do remember that you are expected to use VSP providers. Click on the button "Find a VSP Doctor" to locate a participating provider. In the Athens area, eight providers are available.
Dental coverage The student accident and sickness plan does not include dental coverage. The Ohio University Student Injury and Sickness Plan will only cover dental services necessary to repair an injury to a sound natural. Students who do participate in either of the two medical plans are eligible to purchase an individual plan of coverage for dental. The enrollment form for this plan is available in the student insurance office located in Room 233 of Hudson Health Center (740-597-1816).
Global emergency assistance Through participation in the Ohio University insurance plan, each insured student is eligible for global emergency medical assistance services when traveling 100 miles or more from his/her permanent home or campus address or abroad. Services are accessible 24 hours a day, 365 days a year and are provided by Scholastic Emergency Services. Services include evacuation, repatriation and return of mortal remains. Once the participant is ready to be released from the hospital, Scholastic Emergency Services will arrange and pay to transport the participant to his/her residence or rehabilitation facility, with medical supervision, if necessary.
