Claims Checklist

Please use the following checklist as a convenient guide to properly submit your insurance claims.  
Gather all your SHS service bills (the invoices with CPT and ICD-9-CM codes), SHS Pharmacy bills, other physician bills, and/or other pharmacy bills.  (Insurance companies need the actual listing of the services provided.  They will not accept your charge receipt or your Ohio University account statement.)
  1. Obtain a claim form from your insurance company.  If you are covered under your parents' plan, contact them to get the appropriate information on submitting medical service charges for reimbursement.  For the Ohio University Accident and Sickness Plan, see the information below on submitting a claim.

  2. Complete the claim form, making sure that you have included all requested information.

  3. Many claim forms have a box for your signature that indicates that the physician should be paid.  For all medical services received at SHS you will be charged through your University account.  You are responsible to pay your bill at Chubb Hall.  Do not sign the box that indicates that the physician should be paid.  The insurance company should reimburse you or your parents (or the policy holder).

  4. Sign and Date the claim form.  

  5. Use a paperclip or staple to attach all bills and supplemental information to the back of the claim form.

  6. Mail all documents to the address provided by your insurance company specifically for claim information.
