revised paid time off form
University Human Resources has revised the Paid Time Off forms for faculty and administrative staff. To facilitate gathering information, the forms for administrators and faculty have been simplified to provide only relevant information needed for reporting time off. You will note some items have been deleted and are not required, specifically providing information about 'nature of illness or injury.'
For your information or reminder, as of June 2009, the paper form for Classified Staff sick time is no longer required and was eliminated for those classified employees reporting time on Workforce.
