Benefit Forms Due June 1, 2007 The deadline for submitting 2007-08 benefit forms is Friday, June 1, 2007. Confirmation statements were mailed to employees earlier this week.
Those with no changes may simply sign and return their confirmation statement to Human Resources. A new enrollment formmust be submitted by those who wish to change coverage. Any changes made are effective July 1, 2007. forms
