Benefit Forms Deadline Friday 5/15/09
Benefit Open Enrollment and Educational Benefits forms are due by 5 p.m. Friday, May 15th. All employees should return their Benefit Open Enrollment form to Human Resources. Educational Benefit forms are required only for those who have a qualified dependent enrolling in classes in the Fall.
Benefits Open Enrollment
Enrollment packets were mailed to all benefits eligible employees in early May. Packets were sent via campus mail. Visit the Open Enrollment website for more details.
Educational Benefits
If your spouse, child and /or domestic partner plans to attend Ohio University in the Fall of 2009, please submit an Educational Benefit Request Form for Qualified Dependents (2010) by Friday May 15, 2009. If you have already submitted this form for your spouse, child and/or domestic partner for the upcoming academic year (Fall 2009 – Summer 2010), there is no need to complete another one.
