Educational Benefit Forms Due 5/15/09
If your spouse, child and /or domestic partner plans to attend Ohio University in the Fall of 2009, please submit an Educational Benefit Request Form for Qualified Dependents (2010) by Friday May 15, 2009. If you have already submitted this form for your spouse, child and/or domestic partner for the upcoming academic year (Fall 2009 – Summer 2010), there is no need to complete another one.
Educational Benefit forms are available in both pdf and Word format from the Human Resource website: http://www.ohio.edu/hr/forms.cfm
Forms are due annually for spouses, children, and/or domestic partners and quarterly for employees.
To verify the application of credit to your account, use the Ohio University Current Account Statement online at: http://www.ohio.edu/finance/bursar/eAccount.cfm (students will need their oak ID and password to access this system). Please note: credits will not appear until charges are applied to the account.
