Educational Benefit Forms Due 5/15/07

If your spouse, child, and/or domestic partner plan to attend Ohio University in the Fall, an Educational Benefits form must be submitted to Human Resources by May 15, 2007.

The 2008 Educational Benefit form for Qualified Dependents (i.e. Spouse, Child, Domestic Partner) is available online: http://www.ohio.edu/hr/upload/Ed_Ben_Qualified_Dep_08.xls

Forms are due annually for spouses, children, and/or domestic partners and quarterly for employees.
