New Employment Form – Homeland Security/DMA

All new employees (including students) hired as of April 14, 2006 will need to complete as part of their new hire paperwork a “Homeland Security/DMA” form.  The form’s purpose is to allow new employees to deny or admit to association with a terrorist group as outlined by the Ohio Department of Homeland Security.

Human Resources will include this form as part of the orientation program. Departments hiring students, group 3’s or other employee types that do not attend HR’s orientation will need to obtain a copy of the form at http://www.ohio.edu/hr/forms.cfm and including the new form along with the other new hire paperwork. The Payroll Department will be keeping this form on file.

Refusal to complete the form or answering in the affirmative as to an association with a terrorist organization will result in a report being made to the Ohio Department of Public Safety and Ohio University Legal Affairs.
