New ID Cards Mailed
Anthem Blue Cross & Blue Shield and EnvisionRx Options mailed benefit identification cards to employees' homes this week. The new cards should be used beginning July 1, 2006.
Employees should update information with their provider during the next office visit by showing the Anthem ID card; the EnvisionRx Options ID card should be presented to the pharmacist when obtaining retail prescriptions after July 1, 2006.
To request additional medical ID cards you may call Anthem at 1-800-599-6903; for prescription ID cards call EnvisionRx Options at 1-800-361-4542. These phone lines will be activated on Monday, July 3, 2006.
