2012-13 Educational Benefits Forms Available

If your spouse, child and /or domestic partner plan to attend Ohio University in the fall of 2012, please submit an Educational Benefit Request Form, Qualified Dependents 2012-2013. Timely submission of this form can ensure correct billing. The Educational Benefits Credit will be applied to your account automatically and will adjust according to the number of hours enrolled.

Educational Benefit forms are available online from the Human Resource website: http://www.ohio.edu/hr/forms/index.cfm

Forms are due each semester for employees and annually for dependents.
