Administrative and Classified staff who plan to retire by July 1, 2006 and receive the $10,000 Early Retirement Incentive Plan (ERIP) bonus must submit the ERIP Statement of Intent Form to Human Resources by 5pm on Friday, April 28, 2006. Employees who submit forms after April 28, 2006 will not be eligible to receive the $10,000 bonus.
This form is required even if paperwork (Application for Retirement, etc.) has been completed directly with the Ohio Public Employment Retirement System (OPERS).
Please note: employees with May 3, 2006 OPERS appointments on the Athens campus are also required to have this form submitted by April 28, 2006.
